How to Fill a Medical Certificate: Field-by-Field Guide with Examples

Filling in a medical certificate correctly matters more than most doctors and patients realise. A certificate with any field missing or incorrectly completed is often rejected, causing significant inconvenience. This guide covers every field of a standard Indian medical certificate with examples of what correct entries look like.

The Standard Indian Medical Certificate: Fields Explained

While formats vary across clinics and hospitals, the standard Indian medical certificate contains these fields:

Field 1: Clinic/Hospital Header

The pre-printed letterhead or manually written clinic details. Should include the clinic name, full address, phone number, and ideally the clinic registration number. Example: Dr. A.K. Sharma's Clinic, 24 Civil Lines, Allahabad - 211001. Ph: 0532-XXXXXXX

Field 2: Date

The date the certificate is being issued. This should be the same as (or close to) the examination date. Format should be: 22/03/2026 or 22 March 2026. Do not use ambiguous formats like 03/22/26.

Field 3: Patient Name

Full legal name, matching the patient's Aadhaar or government ID. Example: Rajesh Kumar Verma. Do not use nicknames.

Field 4: Age and Gender

State age in years: 32 years. For children, use years and months: 8 years 4 months. Gender: Male / Female / Other.

Field 5: Diagnosis

This is the most medically important field. State the clinical diagnosis using accepted medical terminology. Examples:

  • Acute viral fever with myalgia
  • Upper respiratory tract infection
  • Migraine with aura
  • Closed fracture of right tibia (undisplaced)
  • Acute gastroenteritis

Continuing the Fields

Field 6: Rest Period

This is the most common source of rejection. The rest period should state: from [specific date] to [specific date], both days inclusive. Never write just "for 3 days" — this is ambiguous. Correct example: from 20 March 2026 to 22 March 2026, both days inclusive.

Field 7: Advice / Recommendation

Standard phrasing: The patient is advised complete rest at home during the above period and should avoid work/strenuous activity. Some templates include space for dietary advice or follow-up instructions.

Field 8: Doctor's Name and Qualifications

Example: Dr. Anita Mehta, MBBS, MD (General Medicine). Qualifications should match what appears on the registration certificate.

Field 9: Registration Number

The doctor's State Medical Council registration number. Example: MH/87654/2015. This is critical — without it, the certificate cannot be verified.

Field 10: Signature and Stamp

The doctor's signature, followed by the clinic rubber stamp. The stamp should include: clinic name, doctor's name, registration number, and contact number.

Frequently Asked Questions

What if I wrote the wrong date on the certificate?

Cross out the error neatly, write the correction alongside, and initial the correction. Do not use correction fluid (whitener) on a medical certificate — it makes the document look tampered with and will likely be rejected.

Can the diagnosis field be left blank?

No. A certificate without a diagnosis provides no clinical basis for the leave and will typically be rejected. If the patient requests confidentiality, use the general phrase "medical condition requiring rest."

Should the certificate be filled in the patient's presence?

The examination must occur in the patient's presence. The certificate can be filled immediately after. In telemedicine, the consultation is remote but the certificate should be issued contemporaneously with the consultation.

Does the patient's address need to be on the certificate?

Not mandatorily, but some employers' forms require it. If the certificate has space for it, filling it in reduces questions about the patient's identity.
