About These Medical Certificate Templates

The 15+ templates in this library are organised into four distinct format categories, each reflecting standard medical document layout conventions observed across Indian clinics and hospitals. Formal hospital formats are typed, letterhead-heavy documents modelled after NABH-style multi-specialty hospitals — the format most corporate HR departments and insurance companies expect. Handwritten GP/clinic formats demonstrate the typical text variation found in India's most common clinic consultation — a registered general practitioner writing with a ballpoint pen on a pre-printed pad. Specialist clinic formats reference dental, maternity, and paediatric clinic letterheads, which carry department-specific logos and terminology. Multispecialty formats combine typed and handwritten elements, reflecting how large hospital groups in cities like Bengaluru, Hyderabad, and Chennai produce their outpatient certificates. The text rendering engine produces per-character variation, including slight rotations to produce clear, natural-looking educational layouts. Format choice should depend on the purpose: a corporate submission needs a formal typed format, while a student absence note may use a simpler GP pad style.

Which Template Should You Choose?

  • Formal-cert / hospital formats — ideal for visualising MNC/corporate submissions, insurance documentation mock-ups, and government sector templates where a typed, structured letterhead is expected.

  • Handwritten GP formats — best represent casual sick leave notes, small clinic submissions, and the everyday doctor's note that most Indian employees encounter.

  • Specialist formats (dental, maternity, paediatric) — use when simulating condition-specific leave documentation, where the clinic type appears on the letterhead and adds specificity.

  • Compact formats — suited for student absence documentation, quick UI wireframe fill-ins, and cases where the certificate needs to be concise rather than formally structured.

Template Fidelity: Research Behind the Designs

Each template design was informed by observing the layout conventions of prescription pads and outpatient certificates across different clinic types that are common across India — general practitioner clinics, government hospital OPD departments, dental clinics, maternity and gynaecology clinics, paediatric clinics, and multispecialty hospital chains. Specific layout elements observed and referenced include: varied letterhead layouts (horizontal two-column vs. centred single-column), different stamp placements (bottom-right vs. bottom-centre vs. overlapping signature), variable vs. typed patient detail fields, and NMC registration number positioning (within the stamp vs. in the letterhead header). No real doctor's name, registration number, or clinic identity was used in creating these templates — all identifying information in the templates is entirely fictional.
